How to Use Your UHC Benefits at Living Well

Getting started with your United Healthcare plan at Living Well is simple:

  1. Contact us. Call (239) 940-7025 or submit a contact form to request your first appointment.
  2. We verify your Optum benefits. Our team contacts Optum to confirm your behavioral health coverage, including your copay, deductible, coinsurance, and any prior authorization requirements.
  3. We share your cost details. Before your first session, we let you know exactly what your financial responsibility will be.
  4. Attend your appointment. Bring your UHC insurance card and a photo ID. We bill Optum directly so you only pay your portion at the time of service.

Understanding UHC Plan Types

United Healthcare offers several plan types, and your out-of-pocket cost for therapy depends on which one you have. PPO and POS plans typically offer the most flexibility, allowing you to see any in-network therapist without a referral. HMO plans may require a referral from your primary care physician before seeing a specialist, though many UHC HMOs exempt mental health services from this requirement. If you have a high-deductible health plan (HDHP), you will pay the contracted rate until your deductible is met, after which your plan's coinsurance applies. Our team helps you understand your specific plan so there are no billing surprises.
